VitreosHealth Raises $5M in Funding

vitreosVitreosHealth, a Plano, Texas-based provider of an insights platform for population health management, raised $5m in funding.

The round was led by an affiliate of Kinderhook Industries, LLC, a private equity firm with over $1.25 billion of committed capital.

The company has now raised more than $6.2m in total funding.

Led by Jay Reddy, President and CEO, VitreosHealth provides a platform that delivers population risk models and predictive and prescriptive health insights to allow everyone throughout the healthcare continuum to achieve a patient-centric focus and contribute to financial performance. The platform startup by giving insights on predictive clinical risk scores, non-clinical markers, gaps-in-care, and prescriptive care recommendations, then shows care managers, clinicians, administrators and C-suite, which patients and members are going to become high risk, predict their probability of hospitalization or escalation of chronic disease, and helps determine the interventions they need right now to delay or avoid high-cost healthcare episodes.
