Cordata Healthcare Innovations, a Cincinnati, OH-based provider of specialty care coordination platforms, closed a $3m Series A financing round.
The round, which included the conversion of outstanding convertible notes, was led by Chicago-based First Health Capital Partners (FHCP), with participation from CincyTech and CVH Holdings.
Cordata also announced the addition of Michael Collette, founder of Patient Point and Executive in Residence at CincyTech, and Caprice Turner of CVH Holdings to its Board. In addition, the Cordata Board has elected Jon Phillips as chair.
The company intends to use the funds to expand the capabilities of its multi-specialty platform for care coordination.
Led by CEO Gary Winzenread, Cordata provides a specialty care coordination platform for healthcare organizations to treat people with chronic specialty diseases and generate better clinical and business outcomes through patient management and retention along complex care paths. The company’s solutions are used by physicians, nurse navigators, service-line directors and business executives at 100+ hospitals and health systems.
