Fold Health, a San Francisco, CA-based provider of a platform purpose-built to support needs of value-based primary care, raised $6M in funding.

The round, which brought the total amount to $12M, saw participation from Iron Pillar and angel investors.

The company intends to use the funds to continue scaling its platform and to hire talent.

Led by CEO Abhijit Gupta, Fold is an outcomes optimization platform purpose built to charge primary care results.  It partners with value-based primary care groups, ACOs, health systems, health plans and digital health companies, to improve the reach and effectiveness of collaborative cares teams in their mission to improve outcomes. Its platform offers an integrated suite of capabilities including: 

  • ClinOps Efficiency Engine: A workflow engine that automates the majority of the work required in value-based care – including care gap closure, transitional care management, annual wellness visit completion, high risk patient engagement, and attribution growth and protection. 
  • Care Team Enablement Suite: Unifies patient data, task management, omni-channel communications and scheduling to keep complex collaborative care teams in tight coordination.
  • Patient Enablement Suite: A fully white labeled patient web and app experience that allows patients to conveniently work with their care team, including self scheduling, chart access, notifications management, and wearables/RPM data integration.
